Medical Evidence Assessment
Everyone starts here. Our comprehensive medical record evaluation forms the foundation for your nexus letter. Every successful claim begins here.
At its core, a medical evidence assessment is a careful, expert analysis of all your medical and service records to evaluate how your health conditions relate to your military service. It’s not just a glance-through. A high-quality chart review: - Assesses every relevant record: service treatment records, VA medical files, private and post-service medical visits, imaging/lab reports, specialty notes, etc. - Identifies gaps, inconsistencies, or under-recognized evidence (for example, symptoms documented in private care that weren’t fully considered in your VA exam). - Determines whether the medical evidence supports a service connection (or aggravation) under VA standards. - Helps frame and organize the most persuasive arguments In short: a chart review is the foundation — the forensic work — that builds the medical backbone of your claim. Without a thorough review, important evidence may be missed or misinterpreted, and your nexus letter (or medical opinion) may lack the strength to make a difference.
Nexus Letter / Independent Medical Opinion
Once your assessment is complete, our team can prepare a formal medical opinion that links your condition to your military service.
Each Nexus Letter is written by a licensed provider using clear, VA-compliant medical reasoning supported by current clinical evidence and regulatory standards. We translate complex medical data into language VA examiners understand — ensuring your documentation is clear, credible, and defensible. While no document can guarantee an outcome, we make sure yours has the strongest medical foundation possible.
Disability Benefits Questionnaire (DBQ)
DBQs are official VA forms that records your level of disability. When combined with a nexus letter, the VA can evaluate your condition quickly and fairly (A fully developed claim).
Our providers complete DBQs that are detailed, consistent, and fully supported by your Medical Evidence Assessment, so the information the VA receives is complete, accurate, and clearly connected to your medical record.

Not every case ends with a nexus letter and/or DBQ. Sometimes additional evaluations, discussions, or preparation can make your claim even stronger. These optional services are available after your initial assessment.

Comprehensive
In-Clinic Evaluation
We offer in-person evaluations for veterans at our Florida clinic.
These evaluations are for conditions requiring physical examination, range-of-motion testing, or diagnostic confirmation. An in-person appointment provides the opportunity for direct clinical findings that complement your existing records and strengthen your medical evidence.
Nexus Prep Documents™
Clear, evidence-based guidance on what to include in your claim.
Having the support of your treating provider can add meaningful strength to your VA disability claim — but most healthcare professionals don’t have the time or VA-specific experience to craft a persuasive nexus letter. Our proprietary Nexus Prep Documents™ make the process easier for both you and your provider. We supply clear, evidence-based guidance on what to include, the key medical elements VA reviewers look for, and the latest research relevant to your condition. This resource removes uncertainty, shortens preparation time, and gives your provider the confidence to support your claim with accuracy and clarity.
